We value collaborative care and streamlined communication, offering two referral options available:

an online form sent directly to us, or a print-and-fax option, whichever works best for you.

Option 1: Online Referral Form (Fast and Secure)

What Happens After a Referral

*Referral Review (Within 1–2 Business Days): Our clinical team reviews the referral, attached documentation, and presenting concerns to determine the appropriate level of care.

*Patient Outreach for Scheduling: We contact the patient directly to schedule the initial psychiatric evaluation. If we cannot reach the patient after multiple attempts, we will notify the referring provider.

*Comprehensive Initial Evaluation: The patient completes a full psychiatric assessment, including diagnostic clarification, medication review, and treatment planning.

*Collaborative Treatment Plan: We develop a patient-centered plan and provide updates to referring providers (with patient consent). Additional coordination occurs as needed for safety concerns, medication adjustments, or shared care.

*Ongoing Follow-Up: We offer continued medication management, supportive guidance, and coordination with primary care, therapists, and other members of the care team.
